The Questionnaires

AHS 2008 is comprised of a screener and surveys for three age groups:

  • Adults (18 years of age and older), including proxy interviews for frail elderly adults;
  • Adolescents (ages 12 to 17), who were interviewed directly with permission of a parent or guardian; and,
  • Children (birth to 12 years of age), for whom AHS interviews the adult in the household who is most knowledgeable about the child’s health.


Topics and questionnaires were developed through a collaborative process designed to balance the information needs of community-based organizations, policymakers and researchers with consideration for the burden on respondents, effect on response rates and available funding. Final questionnaire design resulted in adult interviews that averaged 30 minutes, child interviews that averaged 15 minutes, and adolescent interviews that averaged 12 minutes.

Initiate a Data Request

  1. Complete the form below. Upon receipt of the confirmation email,
  2. Provide a resume or curriculum vitae of the person(s) primarily responsible for data security and analysis; and,
